Professional Geriatric Care Managers assist elderly adults and their families in finding and managing appropriate resources and healthcare services.

There are 4 Core Areas that affect the quality of life for our elderly clients.  Our services are designed to work together in all Core Areas:

Health and Well Being

• We begin by conducting a comprehensive evaluation and preparing a written care plan report with services and steps to take suited to your individual needs.

• We go with you to the hospital or doctor's appointment as your medical advocate.
• We make arrangements for the services you need…such as homemaking, personal care, transportation, nursing and therapy services, home repair, or yard work.
• We monitor the services you are using, whether at home or in a facility, to make sure you are truly getting the help you need.

Financial Resources

• In our care plan report, we tell you about ways to make your resources last and benefits to which you might be entitled, and we help you apply for those benefits.
• We refer you to professionals such as tax consultants, elder law specialists, and long term care financial planners to maximize the resources that you have.

Legal Documents

There are 4 legal documents that everyone needs:

• Durable Power of Attorney
• Designation of Health Care Surrogate
• Living Will
• Last Will and Testament

If you don't have these documents, or if your documents are 10 years old or more, then you should consult with an elder law specialist...and we'll refer you to the best!

Residential Choices

• Our focus has always been to help you stay at home safely for as long as possible. When it becomes too hazardous or too expensive to stay at home, we'll help you find the best senior community, apartment, assisted living or nursing home suited to your     needs and financial resources.
• Once you've selected a new home, we take care of all the arrangements. We'll take care of all the paperwork, we have people to help you pack, hold an estate or yard sale, as well as move and unpack.  We even find a great broker to sell your home!
