There are very few seniors who look forward to moving from their family home, and especially not to a care facility! But when we explore what's behind the question, we learn what the real concerns are.

Discharge Planning
If you've been to the hospital recently, then you know how complicated and confusing healthcare has become. Who's the attending physician? Who's the admitting doctor? What tests are being done and why? When will we know something? And after treatment or surgery, are you surprised to learn at the last minute that your family member is being discharged? Do you know what kind of place they're sending your loved one to? Have you been a part of anything that happened at the hospital? Did your experience at the hospital make you feel helpless?

While your loved ones are in the hospital.
We work with the care manager or discharge planner to plan for the care they need when they leave the hospital. We help you choose the best rehab or arrange skilled care at home. Our goals are to make the process as easy and understandable as we can while ensuring that your seniors are getting the care they need.
Finding a New Home
We know that moving from home is one of the most challenging decisions an older adult might make in his or her lifetime. Typically, we help clients move who need more care than they can afford at home. Round the clock home care may cost more than $10,000 per month. That's right….per month. Assisted living facilities, who provide 24/7 care, cost $2,000 per month to $5,000 per month. But do they need assisted care, something more independent or more skilled care?
We are experts in this field! We stay current with the owners, staff, and licensing of all care facilities in Northeast Florida. When we do our assessment, we evaluate your loved ones needs and financial resources and match them with the facilities and services that will meet their needs. Then we give you a written report so you can compare the choices you have to provide more services for your loved ones at home, or check out the care facilities that we recommend.
Here are some of the areas that we cover in our assessment:
1. How do you cope with change?
- Do you turn lemon into lemonades?
- Or do you respond to change with dread or denial?
2. What is the quality and quantity of your social supports?
- Do you maintain contact and intimacy with old friends?
- Do you have honest and frequent communication with family?
- When you need help, who do you count on?
3. How would you rate your overall health?
- What is your medical history and current condition(s)?
- What kind of help, if any, do you need with ADL's?
How would you rate your mobility?
- How well do you take care of your own needs and what do you ask others to do for you?
4. Financial Resources and Legal Documents
- What assets and income from all sources do you have?
- What are your expenses and debts?
- Have you made a will and other estate plans, created a durable power of attorney and designated a health care surrogate?
Once a decision to be move has been made, we help you with all the services and people needed to make the move as comfortable as we can. From packers and movers, to estate sales and realtors, we can arrange for and coordinate all the help you need. That's why we call it The Right Move. |

As important as it is to know what to expect when your relative enters the hospital, it is just as essential to know what happens when he is ready to leave. The Family Discharge Planning booklet is a guide to the discharge planning process. Because you will continue to be involved in your relative’s care, you need to know about what happens next.
