~ Medication Reconciliation ~
Medication errors are increasingly noted as a problem, both in healthcare publications and the media. Most are preventable if drug reconciliation is in place. Also, reconciliation of medications can lengthen an elderly person's life and increase the quality of their life.
We provide our clients with a consult by a licensed pharmacist who will review all prescription medications and all over the counter drugs from all of their physicians.
The pharmacist will advise us if there are contraindications, duplications, or improper dosages. We will report the pharmacist’s recommendations to our client’s physicians for any follow up changes to be made.

They fill traditional prescriptions as well as custom compounded medications and also provide other services such as detailed, in-depth counseling sessions and drug regimen reviews.
WELLHealth Rx pharmacists, Rakesh and Hemali Patel, work closely with prescribers to ensure that the correct medication at the correct dose and frequency are prescribed. They also work with patients and physicians to provide a cost effective medication regimen.
Rakesh and Hemali have extensive training and experience in geriatric pharmacy and have worked with physicians and patients to provide the most appropriate medications for seniors.
Rakesh Patel, R.Ph, C.Ph, MBA
Rakesh Patel received his Bachelor of Pharmacy degree from Southwestern Oklahoma State University and also has a Masters in Business Administration. He has 9 years of experience working with medication regimens for long term care residents as a consultant pharmacist and was a Regional Consultant Manager for 2 of those years. Rakesh also worked with Siemens for 2 years implementing pharmacy and medical technology in hospitals over the country before opening WELLHealth Rx.
Hemali Patel, Pharm.D.
Hemali Patel received her Doctor of Pharmacy degree from the University of Georgia and completed an ASHP (American Society of Health Systems Pharmacy) accredited Pharmacy Practice Residency at Shands Jacksonville (formerly University Medical Center). She is trained in hospital pharmacy and critical care medicine and also has experience in the retail pharmacy setting as well as counseling patients about their medications. |
